Terms of service

These are a plain-language summary of how the service is intended to work. They are not a substitute for a signed agreement, and a signed agreement governs where the two differ. If you are evaluating this for a medical practice, ask us for the full terms and the business associate agreement before you rely on anything here.

What we provide

Hosted forms that collect information on your behalf, a console to build and manage them, and the records showing what was collected and who read it.

What you are responsible for

That you are entitled to collect the information you ask for.
Keeping your accounts and second factors secure. We cannot tell an authorised sign-in from one using stolen credentials.
Where you send information after we collect it. We block what we can; a destination you configure and control is yours.

Agreements

Where we handle patient information on behalf of a covered entity we do so as a business associate, under a signed agreement. Collection stays switched off until the agreements your arrangement requires are countersigned. That is enforced by the system, not by policy.

Your data is yours

You can export it at any time, in a usable format.
On termination you can take it, and we will destroy our copies on request.
We do not use it for anything except running this service for you.

Availability and limits

We aim to be available and we will tell you when we are not. Any specific commitment lives in the signed agreement, along with the liability terms — not in a marketing page, where it would not be worth much.
